We are about to do a mailout for a fundraising campaign to our church. Is there a way to export the family name with the address?

Yep!
You will want to read the Manual Section on Merge Documents. There are some samples that are included with a default installation of Rock to help you get started, but you may want to duplicate those and make changes.
