Questionnaire Forms

We currently use Church Community Builder but are in the process of switching to ROCK. One feature we use a lot to gather information from our people is Forms. We have several questionnaires we have people complete that stick with their profile. I am sure there is a way to create these in ROCK, I just can't find it. Help! How do I create various questionnaire forms that can be linked to our website for people to complete?

    I believe you can accomplish this with a workflow that presents a form to the user, whose fields are then saved to person attributes representing those fields. The gist of making this work would look something like:

    1) create the necessary person attributes and optionally add them to a new category that represents the specific questionnaire (this keeps the attributes "together" as well). 

    2) Create a workflow that presents a form (use the built in IT request as an example) and configure the form's fields to save to workflow attributes, and then add an action to save the attributes to your questionnaire person attributes.

      Just circling back to let you know there is a new Lava To PDF workflow action for sale in the Rock Shop. It lets you easily create a user input form that when submitted formats the form fields however you want (HTML/lava) and then saves it as a PDF document attribute on a persons record. Works really well.
